The Idaho Name Change Process
1. Your Social Security Card
The first agency to notify of your new name is the Social Security Administration. Complete the SS-5 form and then decide if you want to file it in person or by mail. You will need file with a certified marriage certificate and your current driver’s license or your U.S. passport. No processing fee will be charged.
Roughly two weeks after your form is processed, you will receive a new SS card in the mail. The card will show your new married name, but your SSN will stay the same. One less thing to memorize!
2. Your Tax Returns

4. Your Idaho Driver’s License
