The Louisiana Name Change Process
Congratulations on your nuptials! Changing your name in Louisiana is as exciting as trying to decide whose turn it is to do the dishes. In the vibrant parishes of Louisiana, from the bustling streets of New Orleans to the serene bayous of Lafayette, the process requires a few steps. Start by obtaining certified copies of your marriage certificate from the county clerk’s office where your marriage was recorded. Next, head over to the Social Security Administration to update your Social Security card. Once that’s done, the Louisiana Office of Motor Vehicles awaits your visit for a new driver’s license. Don’t forget to inform other institutions like banks, credit card companies, and the post office. 1. Social Security Administration
The first step to changing your name in Louisiana is filing the SS-5 form. Simply complete the form and file it with your certified marriage certificate and either your U.S. passport or driver’s license. The SSA does not charge a fee to process your name change request.
Two weeks after you file, your new SS card will arrive in the mail. Your card will show your new married name, but your SSN will remain the same.
2. Internal Revenue Service
Next up is changing your name with the IRS. Fill out the IRS 8822 form and then mail it to the appropriate IRS office location with a photo copy of your marriage certificate. *You are not required to file this form with the IRS, but it’s free and ensures that the office has your new name on file when they issue tax returns. We believe it’s totally worthwhile!
3. United States Passport
Step number three involves filing for you new married name with the State Department. Your passport status determines if you should complete the DS-11, DS-82 or the DS-5504 form. You have the option to file the DS-11 and DS-82 by mail. But, the DS-5504 must be filed in-person at a Passport Agency office.
When filing for your new passport, be sure to include your completed form with a certified marriage certificate, two 2×2 photos, and the correct processing fee. Your passport will arrive in the mail 5-6 weeks after filing your form. So, plan your international travel with that gap in mind!
4. Louisiana Driver’s License
Once you’re done updating your name with the federal government, it’s time to head to the Louisiana DMV. You will need to bring your current driver’s license, certified marriage certificate, proof of insurance, and $13 in cash to file for your new license. The best time to go to the office is mid-month and mid-week to avoid waiting in line. Your new license in your married name will be issued the same day you file!
5. Louisiana Voter Registration
Changing your name on your Louisiana voter registration is very important. You will need to complete your state’s Voter Registration Application form and mail it to your County Registrar of Voters. Their addresses are listed on the third page of the form. Once you have filed this form, you can rest assured about your ability to vote under your married name.
6. ALL of Your Creditors and Accounts
Systematically changing your name with your creditors and on all of your personal accounts is the last step of the name change process. Start by notifying your employer and banking institutes. Usually, they will want you to file a form in-person along with your certified marriage certificate. Next, update your name with your credit card companies, mortgage companies, insurance companies, loan providers, utility companies, and professional license boards. Most will change your name over the phone, or after you file an online form. Finally, it is time to put your new name on all of your memberships, periodicals, and of course your social media accounts!
