The Utah Name Change Process
1. Social Security Card
Step one of your name change process is filling out the SS-5 form. Once completed, decide if you want to file by mail or in person. You will need your current passport or Utah driver’s license and your certified marriage certificate for you form to be processed. The Social Security Administration does not have a processing fee!
Approximately 2 weeks after you file your SS-5, your new Social Security card will arrive in the mail. The card will have your new married name, but the same SSN.
2. The IRS
Next up is notifying the Internal Revenue Service of your name change. Filing the IRS-8822 is not required after marriage. 3. U.S. Passport
Step three is changing your name on your passport. The State Department has three different forms based on the status of your current passport. They are the DS-11, the DS-82, and the DS-5504. After selecting and completing the right form, file it with your current passport, your certified marriage certificate, two passport photos, and the listed processing fee.
The State Department usually take 5-6 weeks to be process and issue new passports. So, be sure to take any upcoming international travel into account, before filing to change your name with them.. Airlines do not allow you to travel under a name not on your current reservation, and will require you to re-book your ticket.
4. Utah Driver’s License
To change your name on your Utah driver’s license, bring your current driver’s license, certified marriage certificate, Social Security card, and your U.S. passport or certified birth certificate to the Utah DMV. The fee is $18 if you are just changing your name. If you are also renewing your license, the fee is $25.00.
After processing your information, your new Utah driver’s license will be issued the same day you apply!
5. Utah Voter Registration
Changing your name on your Utah voter registration is the last state-level update you will need to file. Simply complete the mail-in voter registration form and send it to your county clerk’s office.
